2. The Appeal Process The Office of the Social Services Assessment Review Board (SLAB) reviews all appeals related to decision-making that the office receives. If you are appealing a decision made by the Office of the Social Services Assessment Review Board, please complete the Appeal Information Form. Upon completion, one copy of the Appeal Information Form will be forwarded to you. If your appeal is a recommendation, the applicant/client is to sign the Form. The completed Appeal Information Form will be filed with the Tribunal and submitted to the SLAB for review. If your appeal is a rejection, there will be a hearing before the Tribunal. The SLAB's Tribunal will hear the facts of your case and may either issue a decision or refer your case back to the CASH Board in order to review the reasons for the decision. Appeal hearings are subject to change as the process is reviewed. The SLAB will notify the applicant/patient of the date, time and location of the hearing if they wish to attend. The SLAB will also provide an appeal pack with relevant information about the appeal that the SLAB received regarding their case. 3. Where To Submit Your Appeal The appeal and the appeal information form must be submitted with the appeal to the Tribunal. It is the responsibility of Applicant/Client(s), and/or the applicant(s) to ensure that everything is filled out correctly. Your appeal will be returned as undelivered.
